COMPOSITION BY COMPRESSION:

– Isemid 1 mg (2,5-11,5 kg): Torasemide 1 mg.

– Isemid 2 mg (>11,5-23 kg): Torasemide 2 mg.

– Isemid 4 mg (>23-60 kg): Torasemide 4 mg.

PHARMACOLOGICAL PROPERTIES

Torasemide is a loop diuretic of the pyridine-3-sulfonylurea class, also called high-ceiling diuretics. Torasemide has a chemical structure between loop diuretics (such as furosemide) and Cl channel blockers.

INTERACTIONS AND INCOMPATIBILITIES

• Co-administration of loop diuretics and NSAIDs may result in a decreased natriuretic response.
• Concomitant use with NSAIDs, aminoglycosides, or cephalosporins may increase the risk of nephrotoxicity and/or ototoxicity from these drugs.
• Torasemide may antagonize the action of oral hypoglycemic agents.
• Torasemide may increase the risk of allergy to sulfa drugs.
• No pharmacokinetic interactions have been reported after co-administration of torsemide with digoxin; however, hypokalemia may increase digoxin-induced arrhythmias.
• Torasemide may reduce renal excretion of salicylates, increasing the risk of toxicity.
• Care should be taken when torsemide is administered with other substances that have a high capacity for binding to plasma proteins.
• Co-administration of torsemide with substances metabolized by cytochrome P450 families 3A4 (for example: enalapril, buprenorphine, doxycycline, cyclosporine) and 2E1 (isoflurane, sevoflurane, theophylline) may decrease its clearance from the systemic circulation.
• The effect of antihypertensive drugs, especially angiotensin-converting enzyme (ACE) inhibitors, may be potentiated when co-administered with torasemide.

INDICATIONS

Dogs: For the treatment of clinical signs related to congestive heart failure, including pulmonary edema.

CONTRAINDICATIONS

• Do not use in cases of renal failure, dehydration, hypovolemia or hypotension.
• Do not use concomitantly with other loop diuretics.
• Do not use in cases of hypersensitivity to the active substance or to any of the excipients.

ADVERSE REACTIONS

• It has been observed very frequently: Renal failure, increased renal blood parameters, hemoconcentration and alterations in electrolyte levels (chlorine, sodium, potassium, phosphorus, magnesium, calcium).
• Episodic gastrointestinal symptoms such as vomiting and diarrhoea, dehydration, polyuria, polydipsia, urinary incontinence, anorexia, weight loss and lethargy have been frequently observed.
• Other effects observed in studies in healthy dogs at the recommended dose were dryness of the oral cavity mucosa, reversible increases in serum glucose and aldosterone concentrations, decreased urine specific gravity, and increased urine pH.

ROUTE OF ADMINISTRATION

Orally.

POSOLOGY

Dogs:

Initial/maintenance dose: Torasemide 0,13 to 0,25 mg/kg/day, once daily.

In case of moderate or severe pulmonary edema, this dose can be increased, if necessary, up to a maximum dose of 0,4 mg/kg/day once daily.

2,5 to 4: ½ start 1 high dose

>4 to 6: 1 start 1 ½ high dose

>6 to 8: From 1 to 1 ½ start From 2 to 2 ½ high dose

>8 to 11: 1 ½ to 2 start 2 ½ to 3 high dose

                2 mg

>11,5 to 15: 1 to 1 ½ start 2 high dose

>15 to 23 From 1 ½ to 2 start From 2 ½ to 3 high dose

                4 mg

>23 to 30: From 1 to 1 ½ start 2 high dose

>30 to 40: From 1 ½ to 2 start From 2 ½ to 3 high dose

>40 to 50: From 1 to 2 ½ start From 3 to 4 high dose

Doses of 0,26 mg/kg and above should only be administered for a maximum of 5 days.

SPECIAL PRECAUTIONS

• In dogs with acute pulmonary edema requiring emergency treatment, the use of injectable medications should first be considered before starting oral diuretic therapy.
• Renal function, hydration status and serum electrolyte status should be monitored before and during treatment at very regular intervals based on a risk-benefit assessment.
• Torasemide should be used with caution in cases of diabetes mellitus.
• Torasemide increases thirst, therefore dogs should have free access to drinking water.
• The safety and efficacy of the drug for dogs weighing less than 2,5 kg have not been evaluated.

• It does not require any special storage conditions.
• Any part of the tablet should be kept in the blister pack and used for the next administration.
